Making an Appointment

Ms. Kristy Quakenbush is the office manager for the Counseling center. She will assist you in making your first appointment and with fee structuring.

Anyone desiring counseling may schedule an appointment with a counselor for an intake interview. Please call the Counseling Center (765) 288-8862 during regular office hours. Office hours are:

• Monday & Wednesday 9am to 5pm
• Tuesday & Thursday 9am to 8pm
• Fridays 9am to noon.

During the intake interview you will meet with a counselor for about 50 minutes. The counselor will explore your concerns, background information, and what you hope to gain from counseling. Ask the counselor any questions you have about receiving services or confidentiality. The primary purpose of the intake interview is to help you clarify your counseling needs and help us assign you to a counselor that will best fit your needs.

Our Fees: We base our fees on a sliding fee schedule, which is based on your annual income. Clients are encouraged to discuss financial arrangements confidentially with the office manager.

New Client Information: Prior to your first appointment you will be asked to complete the intake forms. For your convenience, you may come by the Counseling Center and pick them up prior to your intake appointment or they can be mailed to your home. Most people take 30-40 minutes to complete these forms.

**Clients who have an urgent need to see a counselor will be seen as soon as possible.
**If you are unable to keep a counseling appointment, please call Kristy to cancel 24 hours in advance.
